Restless Leg Syndrome (RLS)

It is estimated that 10-15% of the general population experience restless leg syndrome [RLS]. This condition has been described as a bothersome sensation producing an irresistable urge to move both legs.

Typical features include:

  1. symptoms are worse at night
  2. inactivity provokes the sensations while activity provides relief
  3. there is associated restlessness which may result in the urge to stretch or pace, and sleep is often fitful with tossing and turning.

There is a significant association with periodic limb movement of sleep [PLMS]. This causes episodic jerking movement about every 20-30 minutes throughout the night. Both RLS and PLMS can lead to difficulty falling and/or staying asleep. Excessive daytime sleepiness often results.

For most people the cause is not clearly understood. It seems a type of movement disorder. There appears to be a hereditary risk. Of the known causes, low iron is quite significant. Deficiencies of folate and magnesium have been reported. Nerve damage from conditions such as diabetes and rheumatoid arthritis, amongst others, can be a factor. A number of medications have been implicated. Dietary influences include caffeine, alcohol and nicotine.

There are several classes of medications that can help. It appears that the class of medications which increase effective dopamine levels[ dopamine agonists or dopaminergic drugs] are most effective. Sedatives, pain relievers and anticonvulsants have also been tried. Pills are best administered 1-2 hrs. before intended sleep.

As medications may be without benefit, only partially helpful, create unwanted side effects, and additional costs, alternative non-drug therapies are always worthy considerations.

Lifestyle factors that have been helpful include:

  1. regular exercise, especially moderate and consistent
  2. good sleep hygiene with a conducive environment, adequate hours and a regular schedule
  3. a stretching / massage program
